It is our pleasure to host you at the third annual SDB Doctors Summit. As we are planning for this amazing event, we kindly ask that you read the entirety of this page, as it contains key information about event registration

For those flying in:

You are responsible for booking a flight that is economy class or equivalent and submitting the receipt for reimbursement through Concur. You will be required to enter your ticket number, carrier, time of arrival, and departure time on the registration page.
You will need to arrive at the hotel no later than 5:00 PM on the day of the event, Thursday, November 14 in Phoenix, AZ.
Transportation to/from the hotel/airport should be booked through Uber/Lyft. It is your responsibility to book this.
Specialty Dental Brands will only reimburse for rides from the airport to the hotel, and from the hotel back to the airport. Please save these receipts to submit for reimbursement on Concur.
Departure flights can’t be booked before 5:00 PM. You will be required to stay for the entire event, which will conclude on Saturday, November 16, 2024, between 2-3 PM.

For those driving in:

You will need to track your total mileage from your office to the event + from the event back to your home office. Please submit your mileage for reimbursement on Paylocity under the expense tab. Your RD can help with this if needed.
You will need to arrive at the hotel no later than 5:00 PM on the day of the event, Thursday, November 14 in Phoenix, AZ.
You will be required to stay for the entire event, which will conclude on Saturday, November 16, 2024, between 2-3 PM. Please do not plan on driving out before then.

Due to hotel policy, all guests are required to leave a credit card on file in case of any incidentals. Specialty Dental Brands is only covering the cost of room and tax for Thursday and Friday nights.

Spouse Inclusion:

Should you choose to bring your spouse, please remember to include their information on the registration form. Spouses will be invited to the Welcome Reception on Thursday evening and the Friday dinner only. Due to the size of our doctor community and agenda structure, we will not be welcoming spouses to join in breakfast/lunches while onsite. The property has lovely amenities which can be enjoyed while you are in session during the day.

Reimbursements:

Finance pays out all approved Concur reports on the 10th and 20th of each month. The money will be funded into your account 3-4 business days after. Please submit your flight receipt or reimbursement right after it is booked for approval.

Registration:

You MUST fill out the registration form to secure a hotel room. We are planning for all attendees to stay onsite, even locals. The deadline for registration is (6/1/2024) We kindly ask that you Do NOT register twice as it can cause errors in our system.
